ATS and ASST

Lombardy is organised differently from the rest of Italy: eight Health Protection Agencies (ATS Città Metropolitana di Milano, Insubria, Montagna, Brianza, Bergamo, Brescia, Val Padana, Pavia) plan and purchase services, while the Territorial Socio-Health Trusts (ASST) run hospitals and community services, alongside public research hospitals (IRCCS) and many accredited private hospitals that deliver care paid for by the public service. GP choice is made online with SPID/CIE or at ASST desks.

Bookings and health record

Appointments and tests are booked on the regional number 800 638638 (landline) or 02 999599 (mobile and abroad), online through the Electronic Health Record and in pharmacies. The Record (fascicolosanitario.regione.lombardia.it) holds reports, prescriptions and vaccinations and lets you pay co-payments and change GP. For out-of-hours care call 116117. The Region decides co-payment exemptions in addition to national ones: check them on the regional portal.

Mountains and community care

In the Alpine provinces (Sondrio, Bergamo, Brescia, Lecco, Como) mountain rescue is coordinated by 112 with helicopter rescue and the National Alpine Rescue Corps; skiers and hikers should check their insurance cover. Community Health Houses (Case di Comunità) for local care have been active since 2023.

Exemptions, co-payments, booking procedures and GP choice change: check the official Regione Lombardia or your ATS website before acting.
